In connection with my participation in the “Poker Power’s Summer Bootcamp” program (the “Program”) sponsored by Poker Power LLC (“PP”):

I certify that I am a United States resident.

I consent to the use of my image, likeness, and sound of voice as recorded on audio or video tape as may be used for PP events from time to time. I understand that my image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right and all claims to royalties or other compensation arising or related to the use of my image or recording. I also understand that this material may be used in diverse settings within an unrestricted geographic area. Photographic, audio or video recordings may be used by PP for any purpose, commercial or non-commercial, including online and other marketing purposes. I understand this signifies that photographic or video recordings of me may be electronically displayed via the internet or in other public settings. There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed. This release is perpetual, irrevocable and binding on me, my heirs and assigns. This release applies to photographic, audio or video recordings collected as part of the Program only.

I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I warrant that I have full right and authority to grant this consent and release. I hereby release any and all liability and/or claims against PP or any of its affiliates, subsidiaries, employees, officers and owners utilizing this material.

I acknowledge and agree that the $100 fee required to enter the Program is non-refundable, non-creditable and non-transferable and is not subject to any warranties or guarantees by PP.

I acknowledge and agree that the Program requires that I play at least fifty (50) hands in the Poker Power application in order to be eligible to enter the final tournament of the Program.

I acknowledge and agree that the prize for the winner of the final tournament is a free ticket to the All-In Summit, which is non-refundable, non-redeemable for cash and non-transferable. I further acknowledge and agree that in the event I am the winner, and unable to attend the All-In Summit, I will notify Poker Power in writing immediately and forfeit the prize, which will be given to the runner-up.
